Dipeptidyl Peptidase-4 (DPP-4) inhibitor
Indication
- Used in the management of Type 2 Diabetes Mellitus
- Helps improve blood sugar control when diet and exercise are not enough
- Can be used alone or in combination with other antidiabetic agents (e.g., metformin, sulfonylureas, SGLT2 inhibitors, insulin)
Mechanism of Action
- Inhibits the DPP-4 enzyme, which breaks down incretin hormones (GLP-1 and GIP)
- Increased incretin levels ? increased insulin release (glucose-dependent) and decreased glucagon secretion
- Results in lower fasting and postprandial blood glucose without significant risk of hypoglycemia when used alone
Dosage
- Usual adult dose: 100 mg orally once daily (with or without food)
- Renal impairment dose adjustment:
- eGFR ?45 mL/min/1.73 m² ? No adjustment
- eGFR 30–44 ? 50 mg once daily
- eGFR <30 or on dialysis ? 25 mg once daily
Side Effects
- Rare but serious:
- Pancreatitis (report abdominal pain immediately)
- Hypersensitivity reactions (Stevens–Johnson syndrome, angioedema)
- Severe joint pain (arthralgia)
Patient Advice
- Take at the same time daily for best results
- Can be taken with or without food
- Maintain healthy diet and exercise plan alongside medication
- Report symptoms of severe abdominal pain, skin rash, or swelling immediately
