Telmisartan + Amlodipine Combination
Category
- Antihypertensive Combination
- ARB (Angiotensin II Receptor Blocker) + Calcium Channel Blocker (CCB)
Mechanism of Action
- Telmisartan: Blocks angiotensin II at AT1 receptors ? prevents vasoconstriction & reduces aldosterone ? lowers blood pressure.
- Amlodipine: Inhibits L-type calcium channels in vascular smooth muscle ? causes vasodilation ? reduces peripheral vascular resistance.
Together, they complement each other: Telmisartan relaxes vessels via RAAS inhibition, and Amlodipine widens arteries directly. This provides better and sustained BP control than either drug alone.
Indications
- Hypertension (when single drug is insufficient)
- Patients needing multi-mechanism BP control
- May help reduce cardiovascular event risk in high-risk patients
Dosage Forms
- 40 mg Telmisartan + 5 mg Amlodipine
- 80 mg Telmisartan + 5 mg Amlodipine
Benefits of Combination
- Dual mechanism gives additive BP lowering
- Reduces BP variability over 24 hours
- Lower risk of dose-related side effects compared to high-dose monotherapy
- Amlodipine’s ankle edema risk is reduced when combined with Telmisartan
Precautions
- Avoid in pregnancy
- Monitor potassium & kidney function
- Use caution in severe hepatic impairment
- Gradually adjust dose to avoid excessive BP drop
